29 CFR 1926.652(a)(1):     Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section except when:    On October 30, 2012, an employee was not provided cave-in protection while working in an unprotected trench measured at 6 feet in depth, exposing the employee to the hazards associated a cave-in.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
