29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    a. On or about November 1, 2012, on the 2nd level of a structure, an employee performing framing operations was not protected from a 13 feet 5-inch fall hazard by use of a guardrail system, safety net system, personal fall arrest system or any alternate fall protection.

29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes:    a. On or about November 1, 2012 on the 2nd level of a structure, an employee performing framing operations was not protected from a fall hazard by use of a fall protection system, in that the walking/working surface was not fully decked.

On or about November 1, 2012, an employee performing framing operations was not protected from a 9 feet 1-inch interior fall hazard, in that his lanyard was rigged to free fall grater than 6 feet.
