29 CFR 1910.146(d)(9):  The employer did not develop and implement procedures for summoning rescue and emergency service, for rescuing entrants from permit spaces, for providing necessary emergency services to rescued employees, and for preventing unauthorized personnel from attempting a rescue:       On or about November 03, 2012, the employer did not ensure procedures for summoning rescue and emergency services, rescuing entrants from permit spaces, providing necessary emergency services to rescued employees or preventing unauthorized personnel from attempting a rescue to eliminate or control the hazards created when a permit-required confined space rescue was needed during welding operations conducted in the elliptical head of a moisture separator unit.

29 CFR 1910.146(g)(1): The employer did not provide training so that all employees whose work was regulated by 29 CFR 1910.146 (permit required confined spaces) acquired the understanding, knowledge, and skills necessary for the safe performance of the duties assigned under 29 CFR 1910.146:    On or about November 03, 2012, the employer did not provide adequate training for employees (supervisors) who were authorized to issue permits for permit required confined spaces.  Employee(s) were exposed to hazard of being caught in a confined space and/or lack of oxygen due to carbon monoxide.
