29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes: On or about November 02, 2012, employees of subcontractor were not protected from falling or stepping through floor holes in the following instances: a) A hole measuring twenty-four (24) inches by eight (8) feet. b) A hole measuring twelve (12) inches by fourteen (14) inches. c) A hole measuring twenty four (24) inches by six (6) feet. Employees were exposed to the hazard of a fall of thirteen (13) feet from the second story to the concrete floor below.
