29 CFR 1926.950(c)(1)(i): No employee shall be permitted to approach or take any conductive object without an approved insulating handle closer to exposed energized parts than shown in Table V-1, unless the employee is insulated or guarded from the energized part (gloves or gloves with sleeves rated for the voltage involved shall be considered insulation of the employee from the energized part): a) On or about October 9, 2012, while working alongside the road way on South Market Road, Marion IL at the Lake of Egypt area, an employee working in a truck aerial lift, installing new transmission lines adjacent to existing 7.2 Kilovolt power lines, was wearing leather work gloves that are not insulated regarding electrical hazards.

29 CFR 1926.952(b)(2): Aerial lift trucks were not grounded or barricaded and considered as energized equipment when working near energized lines or equipment or the aerial lift truck shall be insulated for the work being performed: a) On or about October 9, 2012, while working alongside the road way on South Market Road, Marion IL at the Lake of Egypt area, employee was using the Altec aerial lift that had a missing insulated cover from its structure that insulated the metal structure components from electrical exposure. The lift contacted the 7.2 Kilovolt power line which resulted in the employee receiving an electrical shock.
