29 CFR 1910.134(d)(3)(iv)(B):     The employer did not provide an air-purifying respirator equipped with a filter certified by NIOSH under 30 CFR  par 11 as a high efficiency particulate air (HEPA) filter, or an air-purifying respirator equipped with a filter certified for particulates by NIOSH under 42 CFR part 84 for protection against particulates: (Construction Reference 1926.103)    Employees were required to wear 3M half-mask elastomeric respirators during abrasive blasting operations and the employees' respirators were equipped with improper cartridges (OV) to protect them from particulates.

29 CFR 1910.134(k)(1)(ii):     The employer did not ensure that each employee could demonstrate knowledge of the limitations and capabilities of the respirator:     Employees required to wear half-mask elastomeric respirators to protect them from particulates had OV cartidges on the respirators and did not fully understand the limitations of the respirator when equipped with OV cartidges.
