29 CFR 1926.652(a)(1):  Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section:    a. On or about December 4, 2012, at the above addressed jobsite, an employee was installing a water line in a trench measured at six (6') feet in depth.  The trench was not provided with cave in protection, thereby exposing the employee to cave in hazards.    THE P&M SEWER AND WATER, INC., WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1926.652(a)(1), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 478798, CITATION NUMBER 1, ITEM NUMBER 3 AND WAS AFFIRMED AS A FINAL ORDER ON July 19, 2012, WITH RESPECT TO A WORKPLACE LOCATED AT 1955 W. JEFFERSON, NAPERVILLE, IL 60540.
