29 CFR 1926.20(b)(1): The employer did not initiate and maintain such programs as may be necessary to comply with this part:    a) Jobsite, 2018 Cypress Street, White Oak, PA: On or about December 4, 2012, during roofing activities, the employer did not ensure that a safety program was in place at the site. Deficiencies noted during the inspection included, but were not limited to ladders not set up straight, air hoses being covered with duct tape over holes and lack of personal protective equipment being used where necessary.
