29 CFR 1903.19(d)(1): The employer must submit to the Agency, along with the information on abatement certification required by paragraph (c)(3) of this section, documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.    The employer failed to provide abatement information for the following cited hazards:    a.  Employees were not trained on the hazards associated with pump jack scaffolding.    b.  Employee was using an extension ladder as a scaffolding plank, a purpose for which it was not designed.

29 CFR 1926.451(g)(1):  Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:      On or about December 5, 2012 an employee was working from a pump jack scaffolding at a height greater than 10 feet with no fall protection.
