29 CFR 1926.404(f)(6):     Grounding path. The path to ground from circuits, equipment, and enclosures were not permanent and continuous.    On or about 11/30/2012, employees using Honda generator, model EB11000, had a 100' extension cord, model FT2 with 12-3 wire, plugged in.  The extension cord was missing the ground terminal/prong.

29 CFR 1910.1200(g)(1):     Employer did not have a material safety data sheet in the workplace for each hazardous chemical which they use.    On or about 11/30/2012, employees were using VERSICO G-500 CM Water Cut-Off Mastic and Seam Clean at 1601 South Jefferson, St. Louis, MO 63104; however, they did not have the required Material Safety Data Sheets available for use/reference.
