29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:      At the jobsite located Bank of America, 1200 Fern Ridge Parkway, Creve Coeur, Missouri on or about October 24, 2014, employees were observed installing underlayment on the roof of a commercial building without the use of fall protection.  The fall distance to the ground below was approximately 13 feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:      At the jobsite located Bank of America, 1200 Fern Ridge Parkway, Creve Coeur, Missouri on or about  October 24, 2014, employees were observed using a Werner Portable Ladder for access to an upper landing with the side rails not extended at least 3 feet above the upper landing surface.  The fall distance to the ground was approximately 13 feet.
