On 12/07/2012 at 510 Champ Clark Road, Bowling Green, MO 63334, employees were engaged in foundation repair of a multi-story apartment building and the soil pile was not placed 2 (two) feet away from the face of the excavation. Mid America Piering was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.651(j)(2), which was contained in OSHA inspection number 330264, citation number 1, item number 1a and was affirmed as a final order on 05/10/2012 and closed on 5/23/2010, with respect to a workplace located near 15 Hyde Drive, Troy, MO 63379.

On 12/07/2012 at 510 Champ Clark Road, Bowling Green, MO 63334, employees were engaged in foundation repair of a multi-story apartment building, in an excavation that was deeper than 5 (five) feet and the employees were not protected from a cave-in. Mid America Piering was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.652(a)(1), which was contained in OSHA inspection number 330264, citation number 1, item number 2b and was affirmed as a final order on 05/10/2012 and closed on 05/23/2010, with respect to a workplace located near 15 Hyde Drive, Troy, MO 63379.
