29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes: a) Jobsite, 1414 Ligonier Street, Latrobe, PA: On or about December 14, 2012, employees were working on a commercial, wooden roof with unprotected holes and were exposed to fall hazards from 30 to 15 feet to the ground below. The employees were wearing body belts as fall protection which is not an acceptable practice.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: a) Jobsite, 1414 Ligonier Street, Latrobe, PA: On or about December 14, 2012, employees were using an aluminum extension ladder to acess the roof and this ladder only extended about 24 inches above the roof. The fall hazard to the employees was approximately 15 feet to the ground below.

Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: a) Jobsite, 1414 Ligonier Street, Latrobe, PA: On or about December 14, 2012, employees were working on a commerical roof and were not protected from falling. The pitch of the roof was 10 in 12, 30 feet to the peak, and the fall hazard to the employees was approximately 15 feet from the lower roof to the ground below. WV Home Improvements was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 707818, citation number 1, item 2a and was affirmed as final order on January 11, 2013, with respect to a workplace located at 806 Summit Drive, Pittsburgh, PA 15132.

WV Home Improvements was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.502(d), which was contained in OSHA inspection number 707818, citation number 1, item 2b and was affirmed as final order on January 11, 2013, with respect to a workplace located at 806 Summit Drive, Pittsburgh, PA 15132.
