29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval:    (a) Yard -  An employee used a Caterpillar X00480 forklift with an attachment without the manufacturers prior written approval, on or about December 19, 2012.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered:    (a) Yard -  An employee used a Caterpillar X00480 forklift with an attachment, not installed by the manufacturer, to transport wheel sets.   The employer did not ensure that the truck was marked to identify the attachment and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered, on or about December 19, 2012.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.242(b): Compressed air used for cleaning purposes was not reduced to less than 30 p.s.i.:    (a)  Shop Work Floor, Tracer Lathe Area - An employee used compressed air set at 90 p.s.i. to clean metal chips from a GE Farrel Tracer Lathe and was exposed to struck-by injuries caused by high pressure air, on or about December 19, 2012.     Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
