29 CFR 1926.501(b)(14):     "Wall openings." Each employee working on, at, above, or near wall openings (including those with chutes attached) where the outside bottom edge of the wall opening is 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches (1.0 m) above the walking/working surface, shall be protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system.      Jobsite:  Employees working on second floor were exposed to falls from second floor windows. Tthe end of the building doors was not properlly guarded with either top-rail or mid-rail, exposing employees to falls of 13 feet.

29 CFR 1926.602(c)(1)(ii):     No modifications or additions which affect the capacity or safe operation of the equipment shall be made without the manufacturer's written approval. If such modifications or changes are made, the capacity, operation, and maintenance instruction plates, tags, or decals shall be changed accordingly. In no case shall the original safety factor of the equipment be reduced.        Jobsite:  Crews were using a Lull and replaced missing safety pins with threaded bolts,washers and nuts not approved by the manufacturer for safe handling procedures.

29 CFR 1926.1052(c)(1):     Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail and one stairrail system along each unprotected side or edge:    Jobsite:  Employees working in the buildings and accessing the second floor were exposed to slips trips and falls due to no handrails installed in the units.
