29 CFR 1910.132(a):     Personal Protective equipment was not used when necessary whenever hazards capable of causing injury and illness were encountered:    a) Northwestern Human Services (Sylvan House): Employees (Residential Counselors), while providing resident care were exposed to Cimex lectularius (bed bugs) as bedbugs were detected in the common areas and multiple bedrooms of the Sylan House home on December 13, 2012. The employer did not provide requested personal protective equipment in the form of coveralls to employees and required them to wear and launder personal clothing. On or about December 18, 2012.

29 CFR 1910.141(a)(5):     A continuing and effective extermination program was not instituted where rodents, insects, or other vermin were detected:    a) Northwestern Human Services (Sylvan House): Employees (Residential Counselors), while providing resident care were exposed to Cimex lectularius (bed bugs) as bedbugs were detected in the common areas and multiple bedrooms of the Sylan House home on December 13, 2012. The employer did not implement an effective extermination plan as the bed bug treatment was not scheduled for the site until January 3, 2012. On or about December 18, 2012.
