Bottle Yard: On or about 01/10/2013, partially empty oxygen and acetylene cylinders in multiple transport racks in storage were not separated by a minimum distance of 20' or by a noncombustible barrier at least 5' high having a fire resistance rating of at least one-half hour thereby creating an accelerated fire hazard.

29 CFR 1910.253(e)(5)(iv): Hose connections shall be clamped or otherwise securely fastened in a manner that will withstand, without leakage, twice the pressure to which they are normally subjected in service, but in no case less than a pressure of 300 psi (2.04 MPa). Oil-free air or an oil-free inert gas shall be used for the test. a.) Metro Machine: On or about 01/09/2013, oxygen and fuel gas hoses were not properly tested after couplings were clamped onto the hoses, in that, tool service air at 90 psi was used to test the hoses for leaks creating a potential fire hazard if hoses rupture.

Cord and plug connected equipment and flexible cord sets (extension cords) which remain connected once they are put in place and are not exposed to damage need not be visually inspected until they are relocated. a.) Mid Atlantic Shop 12/27: On or about 01/16/2013, a damaged 50' yellow extension cord was not inspected prior to use in that bare conductors were exposed creating a shock hazard to employees.
