Ladders were used for other uses than that for which they were intended:    (a) While working on a transformer in Hawaiian Electric Company's substation next to the power plant on Ala Moana Blvd. employees used the top step and the top of an 8 foot tall step ladder as a step.

29 CFR 1910.132(d)(1):     The employer did not assess the workplace to determine if hazards were present, or were likely to be present, which necessitated the use of personal protective equipment:      (a)  An assessment for providing a safe means of fall protection was not done prior to commencing work and exposing workers to a fall height of 10 feet.  A point to connect fall protection and the selection fall protection suitable for the working height was not identified and used.
