KATHLEEN WRIGHTWYATT DBA ELITE LLC WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 1910.134(C)(1), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 604398, CITATION NUMBER 02, ITEM NUMBER 001 AND WAS AFFIRMED AS A FINAL ORDER ON OCTOBER 10, 2012, WITH RESPECT TO A WORKPLACE LOCATED AT 1449 MIDDLE ROAD, AVON, OHIO 44011.
