(a) Concrete Mixer Line:  Concete hopper fill station where concrete and steel shot media are manually added to hopper, was not guarded from employees falling into the recessed floor hopper, and from falling onto the unguarded ingoing nip points created by head pully, belt and drive unit of the iron mill scale infeed conveyor.
