29 CFR 1926.501(b)(1):     Each employee on a walking/working surface with an unprotected side or edge 6 feet or more above a lower level was not protected from falling by the use of a guardrail system, safety net system, or personal fall arrest system:    a) 916-924 Green Street- Employees performed demolition activities on the roof of the building where they were exposed to falls of approximately 28' without a guradrail system, safety net system or personal fall arrest system.  Condition observed on October 10, 2012.     NOTE:  Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

Demolition of the exterior brick walls was well underway and the stairway at the East end of the building down to the loading dock area had not been covered.  Conditions observed on October 10, 2012:      NOTE:  Abatement certification and documentation required within 10 days after abatement date. Employees were exposed to being struck by rubble and debris while they worked in and traveled through the area below.  Conditions observed on October 10, 2012. 29 CFR 1926.850(a):     Prior to permitting employees to start demolition operations, an engineering survey was not performed, by a competent person, of the structure to determine the condition of the framing, floors, and walls, and possibility of unplanned collapse of any portion of the structure. The employer shall have in writing evidence that such a survey has been performed:    a)  916-924 Green Street- The manual demolition of the roof and upper floor masonry walls of the structure began on October 8, 2012.  On October 10, 2012 demolition was well underway and the employer could not produce any evidence of an engineering survey that had been performed to determine the condition of the structure. Conditions observed October 10, 2012.    NOTE:  Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. 29 CFR 1926.25(a):     During the course of construction, alteration, or repairs,  debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures:    a) 916-924 Green Street- Employees were exposed to trip and fall hazards traveling through demolition debris from the roof and upper walls which had been allowed to build up on the east end of the second floor.  The rubble and debris obstructed the passageways and stairs used to access the roof. Conditions observed on October 10, 2012.
