29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  a) On or about January 30, 2013, at 17555 El Camino Real, Houston, Texas; employees working on the roof were not protected by a fall protection system. This condition exposed the employees to a fall hazard of 12 feet onto the ground.

29 CFR 1926.150(c)(1)(viii): Portable fire extinguishers were not inspected periodically and maintained in accordance with maintenance and use of portable fire extinguishers, N.F.P.A. No. 10A-1970:  a) On or about January 30, 2013, at 17555 El Camino Real, Houston, Texas; the employer did not ensure that the portable fire extinguisher was inspected prior to arriving at the job site. The extinguisher did not had the security pin.
