29 CFR 1918.62(d)(1): The employer did not adhere to the manufacturers' ratings or use recommendations for the specific synthetic fiber rope or synthetic fiber rope sling used and/or did not have such ratings available for inspection: Port of Stockton, Genco Normandy Vessel, Hold #2: On January 31, 2013, and at times prior thereto, employees were exposed to the hazard of becoming struck and/or crushed while hoisting loads such as, but not limited to, a CAT bulldozer into the vessel's hold with polyester roundslings that were damaged and torn with exposed core yarns.

29 CFR 1918.62(a)(2): Distorted hooks, shackles or similar gear were not discarded: Port of Stockton, Genco Normandy Vessel, Hold #2: On January 31, 2013, and at times prior thereto, employees were exposed to the hazard of becoming struck and/or crushed while using a lifting hook with a twisted safety latch that did not entirely close. The lifting hook, which was attached to a wire rope, had been used to hoist a CAT bulldozer into the vessel's hold and the employer did not ensure the defective gear was removed from service.
