The Shaw Group, Inc., DBA Shaw Modular Solutions,  (Lake Charles, LA) did not record the following workplace injuries correctly or completely on the OSHA 300 Log for calendar year 2012:     a) The employer does not ensure that entries on the OSHA 300 log are complete, accurate and corrections made to any deficiencies identified.  This violation occurred on or about December 31, 2012, when an injury that occurred on February 3, 2012, of employee SMS-12-041, was not included on the OSHA 300 log for 2012.  The employee suffered a back injury when picking up a water cooler in Bay 1 resulting in restricted duties for twenty-three (23) days.      b) The employer does not ensure that entries on the OSHA 300 log are complete, accurate and corrections made to any deficiencies identified.  This violation occurred on or about December 31, 2012, when an injury that occurred on March 16, 2012, of employee SMS-12-011, was not included on the OSHA 300 log for 2012.  The employee suffered a puncture wound to the right leg while inspecting welds on a plasma table and falling through the gaps onto the plasma table spikes causing a puncture wound resulting in restricted duties for eleven (11) days.       c) The employer does not ensure that entries on the OSHA 300 log are complete, accurate and corrections made to any deficiencies identified.  This violation occurred on or about December 31, 2012, when an injury that occurred on June 5, 2012, of employee SMS-12-001, was not included on the OSHA 300 log for 2012.  The employee suffered a back injury when removing clamps from module CA-01-024 resulting in restricted duties for fifteen (15) days.       d) The employer does not ensure that entries on the OSHA 300 log are complete, accurate and corrections made to any deficiencies identified.  This violation occurred on or about December 31, 2012, when an injury that occurred on January 25, 2012, of employee SMS-12-048, was not included on the OSHA 300 log for 2012.  The employee suffered an elbow injury when striking the elbow on a column in the work area resulting in medical treatment beyond first aid.      e) The employer does not ensure that entries on the OSHA 300 log are complete, accurate and corrections made to any deficiencies identified.  This violation occurred on or about December 31, 2012, when an injury that occurred on May 17, 2012, of employee SMS-12-024, was not included on the OSHA 300 log for 2012.  The employee suffered a shoulder injury when a picnic table fell on top of the employee resulting in a tear to the right deltoid muscle and one-hundred eighty (180) days away from work.      f) The employer does not ensure that entries on the OSHA 300 log are complete, accurate and corrections made to any deficiencies identified.  This violation occurred on or about December 31, 2012, when an injury that occurred on May 1, 2012, of employee SMS-12-018, was not included on the OSHA 300 log for 2012.  The employee suffered an eye injury when welding on a baffle plate causing a burn to the left eye and restricted duty for one (1) day.      g) The employer does not ensure that entries on the OSHA 300 log are complete, accurate and corrections made to any deficiencies identified.  This violation occurred on or about December 31, 2012, when an injury that occurred on November 26, 2012, of employee SMS-12-014, was on the OSHA 300 log for 2012 as an Other Recordable Injury rather than Days Away From Work.  The employee suffered a left shoulder and bicep injury that resulted in days away from work for three (3) days.      h) The employer does not ensure that entries on the OSHA 300 log are complete, accurate and corrections made to any deficiencies identified.  This violation occurred on or about December 31, 2012, when an injury that occurred on October 29, 2012, of employee SMS-12-029, was not included on the OSHA 300 log for 2012.  The employee suffered a lower back injury while turning away from another employee?s desk at work, resulting in restricted work duties for nine (9) days.       i) The employer does not ensure that entries on the OSHA 300 log are complete, accurate and corrections made to any deficiencies identified.  This violation occurred on or about December 31, 2012, when an injury that occurred on November 6, 2012, of employee SMS-12-022, was not included on the OSHA 300 log for 2012.  The employee suffered a lower back injury when a clamp failed as the employee was tightening a ratchet strap causing a fall to the floor resulting in restricted work duties for fourteen (14) days.      j) The employer does not ensure that entries on the OSHA 300 log are complete, accurate and corrections made to any deficiencies identified.  This violation occurred on or about December 31, 2012, when an illness that occurred on November 1, 2012, of employee SMS-12-032, was not included on the OSHA 300 log for 2012.  The employee suffered flash burns to both eyes while verifying weld quality on the Gantry welder during operations resulting in restricted duties for one (1) day.      k) The employer does not ensure that entries on the OSHA 300 log are complete, accurate and corrections made to any deficiencies identified.  This violation occurred on or about December 31, 2012, when an injury that occurred on December 27, 2012, of employee SMS-12-045, was not included on the OSHA 300 log for 2012.  The employee suffered a 1.5 to 2 inch laceration to the right hand while sharpening a stainless steel scraper resulting in medical treatment beyond first aid.
