(b)  Inside the building under renovation on or about February 11, 2013, a 2 inch by 4 inch handy box was not used as designed to be used as it was hung by wires from an electrical panel that powered a 110 VAC outlet in the box.  The box was designed to be mounted on a surface.

29 CFR 1926.403(i)(2)(i): Except as required or permitted elsewhere in this subpart, live parts of electric equipment operating at 50 volts or more was not guarded against accidental contact by cabinets or other forms of enclosures, or by any of the following means:  (a)  Inside the building under renovation on or about February 11, 2013, live buss bars and circuit-breaker terminals were exposed to contact in a 220 VAC electrical panel box.  The box was 12 inches wide by 16 inches high, and the bottom was 45 inches from the floor.  Employees carried rebar and metal posts for concrete shoring in the area.
