29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder was not secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder: (a) Inside the building under construction on the west wall on or about February 13, 2013, a 13 foot long extension ladder did not extend at least three feet above the landing level and was not secured at its top. There were no devices to grasp onto. Employees used the ladder to access the roof level. They were on the roof installing a metal roof on steel trusses.

29 CFR 1926.1053(b)(20): When ascending or descending a ladder, the users did not face the ladder: (a) Inside the building under construction on the west wall on or about February 13, 2013, an employee descended a 13 foot long extension ladder with his back to the ladder. The employee was on the roof installing a metal roof on the steel trusses.
