29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces shall be protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes:    On or about 2/13/2013, Basement    a) The floor opening measured 3' wide, 8' long and 7' to the basement. The employees placed a 7' wood ladder near the opening to gain access to the 2nd floor. They were exposed to an approximate 15' fall hazard.
