29 CFR 1910.178(l)(1)(ii): The employer did not ensure that each operator had successfully completed the training required by paragraph (l) prior to permitting an employee to operate a powered industrial truck:    a) Thrift Recycling Management, Inc dba Discover Books, Toledo, Ohio:  On or about February 12, 2013, employees operated power industrial vehicles to lift and/or maneuver boxes of books throughout the facility.  The employer did not ensure that employees successfully completed an operator training program containing elements in 29 CFR 1910.178(l) before employees were permitted to operate lift trucks and electric pallet jacks.      Thrift Recycling Management, Inc. dba Discover Books, as successor in interest to Pro Quo Books, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.178(l)(1), which was contained in OSHA Inspection Number 311606792, citation number 1, item number 2 and was affirmed as a final order on March 22, 2009, with respect to a workplace at 830 N. Westwood Ave., Toledo, Ohio.

29 CFR 1910.178(q)(7): Industrial trucks were not examined before being placed in service:    a) Thrift Recycling Management, Inc. dba Discover Books, Toledo, Ohio:  On or about February 12, 2013, the employer did not ensure that powered industrial trucks were examined daily for conditions that could adversely affect the trucks safety before being placed into service.  Employees operated forklifts and electric pallet jacks to move various materials throughout the facility.    Thrift Recycling Management, Inc. dba Discover Books, as successor in interest to Pro Quo Books, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.178(q)(7), which was contained in OSHA Inspection Number 311606792, citation number 2, item number 3 and was affirmed as a final order on March 22, 2009, with respect to a workplace at 830 N. Westwood Ave., Toledo, Ohio.
