(a)  Front of Building Service Bays - On or about February 15, 2013 employees were exposed to fall hazards while elevated on a platform extending 30 inches outside the wheelbase of a rough terrain forklift and they were not protected by their employer ensuring that the platform extend no more than 10 inches.      (b)  Front of Building Service Bays - On or about February 15, 2013  employees were exposed to fall hazards while elevated 12 feet on a platform and they were not protected by a guardrail system with a midrail.
