29 CFR 1910.305(b)(1)(ii): Unused openings in cabinets, boxes, and fittings were not effectively closed:  On or about February 12, 2013, employees were working on Rig # 2 where two electrical boxes had unused openings that were not closed. One was located in front of the company man and tool pusher's trailers on a 480V three phase dry type transformer. The other was located outside the generator room which was used for a 110V battery charger. Employees were exposed to potential electrical hazards.

29 CFR 1910.305(b)(2)(i): In completed installations, each outlet box did not have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass were not provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear:  On or about February 12, 2013, employees were working on Rig #2 where an electrical box had a broken faceplate and another electrical box had cords running through a hole that didn't have a bushing. The first was located on a 480V three pahse transformer in front of the tool pusher and company man's trailers. The other was a 480V electrical box in the electrical room. Employees were exposed to potential electrical hazards.

29 CFR 1910.305(g)(1)(iv)(B): Where run through holes in walls, ceilings, or floors:  On or about February 12, 2013, employees were working on a Rig # 2.  The change room had an electrical cord running through a hole in the metal wall,  it was powering a heater from a 220V outlet. Employees were potentially exposed to electrical hazards.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables shall be connected to devices and fittings so that strain relief is provided that will prevent pull from being directly transmitted to joints or terminal screws:  On or about February 12, 2013, employees were working on Rig #2. An electrical box outside the generator room  had a cord running from it to the inside of the room; it was used to power a 110V battery charger. The cord had a damaged strain relief which exposed employees to potential electrical hazards.
