29 CFR 1910.23(d)(1): Every flight of stairs having four or more risers was not equipped with standard stair railings or standard handrails as specified in paragraphs (d)(1)(i) through (v) of this section, the width of the stair to be measured clear of all obstructions except handrails: On or about February 12, 2013, on the northwest corner of drill site, the stairway railing leading up to the mixing tank did not have a mid-rail, the top rail measured 23-inches to the base. Employees were exposed to a fall approximately 10-feet from the top of the stairs to the ground.
