29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective: On or about February 12, 2013, equipment on location was not grounded effectively in the following instances: 1. Next to the Toolpusher Trailer, the 3-foot ground rod ("electrode") connected to the 480V step-down transformer was in the soil six inches. 2. The 3-foot ground rod ("electrode") connected at the front end of the Bottom Doghouse, was in the soil six inches. Employees were exposed to electrical hazards.

29 CFR 1910.305(b)(1): Conductors entering boxes, cabinets, or fittings were not protected from abrasions: On or about February 12, 2013, next to the Toolpusher Trailer, on the lower left hand-side of the 480V step-down transformer a bushing was missing where the 220V nominal cable was coming out. Employees were exposed to electrical hazards.

In the Bottom Doghouse, a 120V nominal electrical light cable was run through the cutout hole in the wall. 2. In the Top Doghouse, a 120V nominal electrical floodlight cable was run through the cutout hole in the wall. Employees were exposed to electrical hazards.
