29 CFR 1910.132(d)(1)(i):     The employer did not select and have each affected employee use, the types of personal protective equipment that would protect the affected employee(s) from the hazards identified in the hazard assessment:    M/V Courageous (Galley):  On or about 2/13/2013, the cook was not required to use cut resistant gloves while using knives in the galley for food preparation.  This condition exposed the employee to laceration hazards.

M/V Courageous (Flow Scale):  On or about 2/13/2013, and times prior thereto, an employee was working on the Flow Scale conveyor without ensuring that all energy sources were rendered safe by de-energizing and locking out the power to disconnects on the equipment; exposing employees to amputaion or caught in hazards.
