29 CFR 1910.132(d)(1)(i): The employer did not select and have each affected employee use, the types of personal protective equipment that would protect the affected employee(s) from the hazards identified in the hazard assessment: C/P Arctic Fjord (Galley): On or about 2/13/2013, the cook did not have nor was required to use cut resistant gloves while using knives in the galley for food preparation. This condition exposed the employee to laceration hazards.

29 CFR 1915.89(e)(1)(ii) Prior to the authorized employee shutting down machinery, equipment, or systems, the employer did not ensure that affected employees had knowledge of the implementation of the lock out and tags system: (a) C/P Arctic Fjord (Factory Area): On or about 2/14/2013, and times prior thereto, affected employees were not aware of the de-energization of Baader equipment, as they were not tagged or identified on the machine as being locked or tagged out; exposing employees to the release of hazardous energy. (b) C/P Arctic Fjord (Surimi Wash Tanks): On or about 2/14/2013, and times prior thereto, affected employees were not aware of the of the de-energization of the Surimi Wash Tanks, as they were not tagged nor identified as being locked out on the machine or in a visible location; exposing employees to the release of hazardous energy.
