29 CFR 1910.213(d)(1):     Circular handfed crosscut table saw(s) were not guarded by an automatically adjusting hood which completely enclosed that portion of the saw above the table and above the material being cut:    (a) Maintenance repair shop: On or about 27 February 2013 and at times pror, there was a Ridgid Table Saw, serial number Z102037375, which did not have a guard that enclosed that portion of the blade above the table and above the material being cut.

29 CFR 1910.304(f)(1)(iv):     Overcurrent devices for circuits rated 600 volts, nominal, or less, were not readily accessible to each employee or authorized building management personnel:    (a) Maintenance room: On or about 27 February 2013 and at times prior, employees were exposed to trips, falls and the hazards associated with being unable to de-energize equipment in the event of an emergency. The breaker panel was blocked by storage.

29 CFR 1904.29(b)(6):     The employer did not enter "privacy case" in the space normally used for the employee's name on the OSHA 300 form or equivalent.     (a) Heritage Acres Nursing Home: On or about 27 February 2013 and at times prior, the employer entered employee names on the 2012 OSHA 300 Form that resulted in a privacy concern case. The most recent entry was made on 12/3/2012.
