29 CFR 1910.303(b)(7)(iv): There shall be no damaged parts that may adversely affect safe operation or mechanical strength of the equipment, such as parts that are broken, bent, cut, or deteriorated by corrosion, chemical action, or overheating: Induced Air Flotation Tank Control Panel: On or about 30JAN13, employees were exposed to electrical shock and burns when the employer failed to properly repair or replace a corroded electrical box that arced and caught fire in a damp outside location where employees work and interact with the panel during their work shifts.

29 CFR 1910.157(c)(4): Portable fire extinguishers were not kept in their designated places when not being used: Control Room: On or about 27FEB13, a fire extinguisher was on the floor about three feet away from the clearly marked designated location on the wall, exposing employees to delay in locating the fire extinguisher in the event of fire.
