29 CFR 1910.303(b)(2):  Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling  a) Composite Room, 987 Griffin Pond Road, Clarks Summit, PA: An energized electrical outlet box had a damaged and/or dislodged combo toggle switch duplex receptacle that was not properly installed. Employees were exposed to contact with electrical current hazards, on or about March 1, 2013.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation.

29 CFR 1910.305(j)(2)(iv):  A receptacle installed in a wet or damp location was not suitable for the location.  a) Composite Room, 987 Griffin Pond Road, Clarks Summit, PA: Energized electrical outlet boxes, combo switch/duplex receptacles and a duplex receptacle were coated with the residue from a caustic liquid "Curative". Employees were exposed to contact with electrical current hazards, on or about March 1, 2013.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation.
