29 CFR 1926.25(a): During the course of construction, alteration, or repairs, form or scrap lumber with protruding nails was not kept clear from work areas:   a)  At the site located at 2113 Crocker Street, Houston, TX:  On March 4, 2013 debris including lumber from the construction on site with protruding nails was not kept clear exposing employees to slip, trip and puncture wound injuries.

29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:   a) At the site located at 2113 Crocker Street, Houston, TX:  On March 4, 2013 an employee beneath a pump jack scaffold 37 feet overhead where siding work was being done was not protected by a helmet, exposing the employee to a head injury.
