29 CFR 1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration:; Such activities were not performed only by experienced and trained employees selected for such work by the competent person. a) At the site located at 911 West Drew Street, Houston, TX: On March 4, 2013 the employer directed employees to work at a height of approximately 28 feet above a lower level from a Pump Jack scaffold that was not erected under the direction of a competent person resulting in deficiencies including but not limted to deficient vertical pole bracing at the top, missing bracing at the bottom of the poles, and missing guardrails or fall protection on the platform, exposing employees to a fall hazard. b) At the site located at 911 West Drew Street, Houston, TX: On March 4, 2013 the employer directed employees to work at heights up to approximately 28 feet above a lower level from job built single plank wood scaffolds that were not erected under the direction of a competent person resulting in deficiencies including but not limited to missing guardrails, insufficient minimum width of working levels, lumber use instead of planking, no cross bracing to prevent racking, and all components not verified as designed by a qualified person to meet the minimum capacity to support 4 times the maximum intended load applied, exposing employees to a fall hazard.
