29 CFR 1926.451(c)(2)(v):     Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied:    (a) On or about March 12, 2013 at 804 Mulberry, Raymore Missouri 64083, employee was exposed to a fall hazard by standing on plywood , layed across elavated forks of a powered industrial trucks unattached.

29 CFR 1926.501(b)(1):     Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    (a) On or about March 12, 2013 employess were exposed to a fall hazard while working at the edge of a roof surface approximately 16 feet without fall  protection.

29 CFR 1926.1051(b):     The employer did not provide or install all stairway and ladder fall protection systems required by Subpart X and/or did not comply with the other pertinent requirements of Subpart X, before employees began the work.    (a) On or about March 12, 2013 at 804 Mulberry, Raymore Missouri 64083. Employees were exposed to a fall hazard by climbing and  working on a stairway  two stories high with out guardrails.
