29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a) An employee of the contractor was installing shingles and underlay paper atop the peaked roof of a new single-family house at 10 Balzano Court in Edison NJ and was not protected from falls of 22 from the roof, through the use of personal fall arrest system, nets, guardrails, or alternate effective means.        Condition(s) were noted on or about 3/14/2013.    Latyki's Roofing and Siding was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 312864598, citation number 1, item number 2 and was affirmed as a final order on 5/25/2010, with respect to a workplace located at 415 Main Avenue, Wood Ridge NJ.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigied support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.    a) A 28 ft fiberglass extension ladder was provided and used for roof access & egress at the front of a new two-story, single family house at 10 Balzano Ct in Edison NJ.  The side rails extended less than 2 ft above the roof edge; requiring the roofer to crouch when grabbing hold of the rail at the top upon descent from the roof, exposing him to fall hazard of 22'.    Condition(s) were noted on or about 3/14/2013.
