29 CFR 1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface were not used: (a) 27 South Park Ave Longmeadow, MA: On or about March 5, 2013, employees were observed climbing down from the roof to the ladder jack scaffold plank which was more than 2 FT below the roof eve and then continued to climb from that scaffold plank down over the scaffold support member to the ladder supporting the scaffold and a means of access in accordance with this subpart was not used.

29 CFR 1926.501(b)(13):Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system: (a) 27 South Park Ave Longmeadow, MA: On or about March 5, 2013, employees were observed working on this residential roof more than 21 FT above the ground below without being protected from falling off the roof as required by this subpart: CD/Acerra Roofing Company was previously cited for a repeat violation of a similar or equivalent occupational safety and health standard 29 CFR 1926.501 b (10) which was contained in OSHA inspection number311319321, citation number 1, item number1 and was affirmed as a final order on October 30, 2008 with respect to a workplace located at 1755 Boston Post Rd Milford, Ct
