HURON PLUMBING & HEATING, INC. WAS PREVIOUSLY CITED FOR A VIOLATION     OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT  STANDARD, WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 312131766,  CITATION NO. 1, ITEM NO. 1, AND WAS AFFIRMED AS A FINAL ORDER ISSUED ON  07/16/2008, WITH RESPECT TO A WORKPLACE LOCATED AT 3203 ORCHARD PARK  ROAD, ORCHARD PARK, NY 14127.      29 CFR 1926.404(b)(1)(i):     Employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section, or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites.      a) On or about 3/20/13 inside building under renovation at 3620 Sheridan Dr., Amherst, NY:  An employee used a Ridgid Sawzall to cut PVC pipe. The sawzall was connected to 120-volt ac power via a series of two extension cords, the first of which was plugged into a wall receptacle in the building. No GFCI or assured equipment grounding program was being used.    NO ABATEMENT CERTIFICATION REQUIRED
