29 CFR  1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes by covers:  a) On or about 11/22/2024, at the first floor apartment 2A area of the site, Corfu, NY. Shower drain, of 7-inch by 7-inch with a 3-inch drain pipe in the middle,  was not covered to protect employees from tripping in or stepping into or through the hole.  NO ABATEMENT CERTIFICATION REQUIRED
