29 CFR 1910.178(l)(1)(ii): The employer did not ensure that each operator had successfully completed the training required by paragraph (l), except as permitted by paragraph (l)(5), prior to permitting an employee to operate a powered industrial truck:     Throughout the facility  The employer is failing to ensure to ensure that all employees operating powered industrial trucks at the facility are trained in accordance with the requirements of this subpart, which are specifically delineated in paragraph (l)(3).  Employees are being allowed to operate these vehicles without the requisite knowledge to operate them in a safe manner.  Some employees have not accomplished a practical exercise.  The employer is allowing employees, who have not been evaluated and certified as outlined in paragraphs (l)(2), (l)(3) and (l)(6) of this subpart, to operate powered industrial trucks at the facility.

29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven -7 feet or less above floors or platforms were not enclosed:     The employer is failing to protect employees from amputations hazards by inadequately guarding the back side of the chain and sprocket assembly on the auger on the bottom of the bean bin in this area.  Employees are able to come in contact with the in-running nip points of the assembly.  This machinery was not fully guarded to protect employees from potential amputation hazards.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:     Throughout the facility  The employer is failing to ensure that the company has a written hazard communications (HAZCOM) program which contains all of the elements required by this subpart.  The company does not have the written program required by this sub part.  Employees are exposed to chemicals including but not limited to caustic powders and industrial cleaning solvents.
