29 CFR 1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used. Crossbraces shall not be used as a means of access.  WORKSITE: The employees, working from the Alum-A-Pole pump jack scaffold were not provided with appropriate and safe means access and were using a second floor window to egress from the platform.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.   WORKSITE: The employees installing underlayment on a 12 pitch roof were not protected from falls approximately 41 feet to the ground by the use of appropriately installed and utilized fall protection systems/devices.

29 CFR 1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment shall be independent of any anchorage being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee attached, or shall be designed, installed, and used as part of a complete personal fall arrest system which maintains a safety factor of at least two, and under the supervision of a qualified person.  WORKSITE:  The lifeline used by an employee as part of his personal fall arrest system was secured to the anchor point by tying a knot in the lifeline to the attachment point and tying a second knot in the center of the lifeline and securing the snap hook to the knot, exposing the worker to a 41 foot fall.
