On or about February 18, 2013, the employer did not ensure that workers used the slicer guard when cutting vegetables using the Chiba Kogyosyo Co LTD "mandolin" vegetable slicer.  The worker received an avulsion injury to the left thumb.

29 CFR 1904.29(b)(3):     The employer did not enter each recordable injury or illness on the OSHA 300 Log within seven (7) calendar days of receiving information that a recordable injury or illness has occurred:    Location:  At the River City Casino facility at or near 777 River City Blvd, St. Louis, MO  63125.    The employer did not enter two (2) recordable injuries on the 2013 OSHA 300 log within seven (7) calendar days for the following injuries:    a.)  An avulsion injury that occurred on Feb. 18, 2013 when a worker was using a Chiba Kogyosyo Co LTD "mandolin" vegetable slicer.    b.)  An eye injury that occurred on Feb. 28, 2013 when a worker was in the kitchen.
