29 CFR 1910.134(e)(1): The employer did not provide a medical evaluation to determine the employees ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace:  a.) Ridg-U-Rak, Inc.-  The employer had not obtained a medical evaluation for a painter required to use respirators. Painters were required to wear North brand, tight-fitting, air purifying respirators during powder coating operations.

Ridg-U-Rak, Inc.- The employer had not ensured that an employee required to utilize a North brand tight-fitting air purifying respirator, during powder coating operations, was fit-tested prior to initial use.   b.) Ridg-U-Rak, Inc.-  The employer had not ensured that employees required to utilize North brand tight-fitting air purifying respirators, during powder coating operations, were fit-tested annually. Employees were last fit tested September, 2002.

Ridg-U-Rak, Inc.-  The employer had not ensured that employees required to use North brand tight fitting air purifying respirators received respiratory protection training annually. The employer and employees were unable to affirm when training was last received. Training was last provided September 2002.
