Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: (a) On or about April 1, 2013 - 201 Industrial Court, Gadsden, AL, employees working at approximately 20 feet above the ground were not provided fall protection while disposing of roofing materials. (b) On or about April 1, 2013 - 201 Industrial Court, Gadsden, AL, an employee working at approximately 7 feet above the ground was not provided fall protection while installing roofing materials.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: (a) On or about April 1, 2013 - 201 Industrial Court, Gadsden, AL, employees removing roofing materials were not aware of the fall hazards. (b) On or about April 1, 2013 - 201 Industrial Court, Gadsden, AL, an employee installing roofing materials from a secondary roofing system was not aware of the fall hazards. Precision Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.503(a)(1), which was contained in OSHA inspection number 314206848, citation number 01, item number 002 and was affirmed as a final order on 03/08/2011, with respect to a workplace located at 84 Smith Mill Road, Bethlehem, GA 30260.
