29 CFR 1910.178(l)(1)(i):   The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):  At this work place, employees involved in tortilla production process operated powered walk along pallet lifts to move stored supplies, and did not have the required training to operate the powered lifing equipment.

29 CFR 1910.242(b):   Compressed air used for cleaning purposes was not reduced to less than 30 p.s.i.  At this work place, employees involved in floor cleaning operations in the tortilla production area used compressed air at approximately 80 to 95 pounds per square inch (PSI) to blow bits and pieces of masa (dough) from under tortilla processing equipment.

29 CFR 1910.95(g)(6):   At least annually after obtaining the baseline audiogram, the employer did not obtain a new audiogram for each employee exposed at or above an 8-hour time-weighted average of 85 decibels.    At this work place, a new audiogram for each employee involved in tortilla production was not conducted annually since 2010.
